I have just returned from a great Safety Management course in London and gained a great deal of insight into the way in which Safety Management Systems (SMS) are implemented.

Note that these are going to be mandated from next January !

A key element of this is the Incident-Reporting scheme. Corporate Aviation is special in this regard, as you are likely to be operating within a small fleet. How does your Safety Manager treat the incidents, and do he make any attempt to analyse trends and commonly-occuring incidents ? Do they occur with sufficient frequency to make this possible ? Do you share any data with other airline or agency in similar circumstances to get a decent sample size ?

Any inputs appreciated...

The key for this to work is for management at the highest level to create a "no blame" culture. People will openly come forward if they feel they will help others learn from their mistakes rather than to be punished. I have seen this work quite well but it takes time and effort from the Safety team to push for reports, no matter how insignificant they may seem at the time.

I have just been reading about CHIRP (http://www.chirp.co.uk)(Confidential Aviation Incident reporting online for UK CAA). Note that it is confidential rather than anonymous. The identity of the reporter is known but not shared.

On their website they have online reporting forms for all the functional areas: flightcrew, ground staff etc, with different fields to be filled.

Anyone using this ? Is there a European-wide (EASA?) alternative ?
